Clinical Snapshot

Food allergy is an immune-mediated, IgE-driven (Type I) hypersensitivity reaction to a specific food protein: the immune system misidentifies a harmless dietary protein as a threat, and on a later exposure, allergen-specific IgE already bound to mast cells and basophils cross-links and triggers the release of histamine and other mediators, producing symptoms within minutes to about 2 hours of ingestion.[1] This is a fundamentally different process from a food intolerance (e.g., lactose intolerance) or a food-related side effect, neither of which involves the immune system and neither of which is reproducibly triggered by trace amounts the way a true allergy can be.

Food allergy affects roughly 6.7% of Canadians overall by self-report — 7.1% of children and 6.6% of adults — making it a genuinely common condition every community pharmacist will encounter regularly, not a rare one.[2] International reviews describe an "impression" of rising prevalence over recent decades, with some populations now reporting rates as high as roughly 10%.[1]

This platform's Anaphylaxis chapter covers the acute emergency protocol for a severe allergic reaction of any cause — diagnostic criteria, weight-based epinephrine dosing, positioning, adjuncts, and the discharge checklist — in full depth, and this chapter does not repeat that content. This chapter's focus is the broader, longer-term picture that determines whether a patient with food allergy actually avoids that emergency in the first place: accurate diagnosis, natural history and prognosis, day-to-day avoidance strategy, allergen-labelling literacy, oral immunotherapy and other emerging disease-modifying options, and the counseling that keeps an anaphylaxis action plan effective between reactions.
