Clinical Snapshot

Lower urinary tract symptoms (LUTS) in men are voiding, storage, and postmicturition symptoms that commonly arise from benign prostatic hyperplasia (BPH) — a nonmalignant, androgen-dependent proliferation of prostatic epithelial and stromal tissue that begins in the periurethral transition zone and can progress to bladder outlet obstruction. Two mechanisms coexist and most pharmacotherapy targets one or the other: a static component (the physical bulk of an enlarged gland narrowing the urethral lumen) and a dynamic component (alpha1-adrenergic-receptor-mediated smooth-muscle tone in the bladder neck, prostate, and urethra). LUTS is common, but LUTS is not synonymous with BPH — prostate cancer, urinary tract infection, neurogenic bladder, urethral stricture, and overactive bladder unrelated to prostatic enlargement can all produce an identical symptom picture, so ruling out other causes is part of every initial workup, not an afterthought. The goals of therapy are to improve or resolve bothersome symptoms, delay or prevent clinical progression, and prevent the long-term sequelae of unrelieved bladder outlet obstruction — recurrent urinary tract infection, bladder stones, and obstructive uropathy with renal impairment.[1] Current Canadian practice follows the 2022 Canadian Urological Association (CUA) guideline update, cross-checked against the 2023 American Urological Association (AUA) guideline amendment and the 2026 European Association of Urology (EAU) guideline on non-neurogenic male LUTS.[2][3]
