Ontario pharmacy professionals have an upcoming opportunity to connect medication-incident reporting with the people affected by it. In its October 1, 2026 e-Connect, the Ontario College of Pharmacists highlighted a free Institute for Safe Medication Practices (ISMP) Canada webinar scheduled for October 28: There Is a Person Behind Every Prescription.

What the session will cover

Melissa Sheldrick, a patient and family advisor, will discuss lived experience and the conditions that support continuous quality improvement, including psychological safety and patient engagement. Enna Aujla will work through a medication-incident case and discuss learning from the National Incident Data Repository. The official event listing confirms both presenters.

OCP lists a one-hour session from 5 to 6 p.m. and says a recording will be available afterward. Confirm the time zone and joining instructions in your registration confirmation. This is an advance event notice: the planned discussion is not a newly published set of research findings.

Why it is relevant to the dispensary

Pharmasist's suggested focus is the gap between recording an incident and learning from it. A team can use the session to examine whether its discussions lead to a specific, testable improvement, rather than ending when a report is submitted.

Practice guidance for pharmacists

  • Register through the official event page and check the calendar details.
  • Bring one de-identified workflow question; do not disclose patient information in a webinar chat.
  • Invite a colleague with a different role to compare what each of you takes from the session.
  • Plan a short follow-up discussion and identify one improvement to assess locally.
